With time being short, we ask that you print this form and bring it with you to the Jamboree. Please bring a check or the exact payment in cash with you as well.

The street address and phone # above are only used if we cannot reach your e-mail address to confirm your registration or to mail books and/or CDs already paid for but not picked up. Once you submit this form we will usually respond within 24 hours with your acceptance or rejection notice.
